The purpose of the SOF (Statement of Findings) is to:
- summarize the factual findings of the inquiry team
- include any relevant staff behavioral policies and standards
- recommend conclusion(s) based on the factual findings
- include any relevant risk assessments if policies and standards aren’t directly applied
- include all administrative actions
- recommend supportive actions
- provide relevant appendices
- include appropriate safety plan(s)
