SOF

The purpose of the SOF (Statement of Findings) is to:

• summarize the factual findings of the inquiry team

• include any relevant staff behavioral policies and standards

• recommend conclusion(s) based on the factual findings

• include any relevant risk assessments if policies and standards aren’t directly applied

• include all administrative actions

• recommend supportive actions

• provide relevant appendices

• include appropriate safety plan(s)
